Don't Know how to mail my phones for ETF reimbursement

Switched to tmobile end of March. Here it is May and I JUST found out that I had to have my phones mailed in by a certain date. I switched in a local Costco store at their kiosk because they were having an in store promotion on the phones I wanted. The person that helped us make the switch NEVER EVEN said they needed to be mailed by a certain date! He even told us that we would recieve the packaging needed to mail in our "trade-in" phones for the etf reimbursement. So now my phones are being mailed in late... since I also couldnt go into a store to trade them... I have the shipping labels printed out and followed ALL the shipping instructions for them and have them in a usps flat rate shipping box the smallest size with bubble wrap... will this be acceptable because I think I read somewhere that you have to put the phones in the packaging tmobile provides... which they don't because we never recieved such thing! I'm really upset about this because we switched to eventually have a lower bill and EXCELLENT phones... and the signal service and speeds are perfect... but this just erks me! I hope "I" won't be held accountable for getting the phones in late. Wasn't my fault. I'm taking initiative to get them mailed ASAP now that I know! can any tmobile employee or someone else that's experienced this with tmobile tell me the good news that the mail in date can be waved BECAUSE OF SOMEONE ELSE'S IDIOCRACY? I won't ever do cell phone business anywhere but in house ever again! UGH!
