I don't think the issue is lack of spectrum. It's lack of history and priority. Most of T-Mobile was build on foundations laid by earlier GSM carriers, like Omnipoint, VoiceStream, Aeriel and PowerTel. In the case of the Carolinas the local GSM carrier was BellSouth that got merged into Cingular and then became ATT. For many years T-Mobile had no service in those states and has had to build what they have from scratch.
